All completed application form should be submitted along with the following documents:-

1.  Photocopy Identity Card of applicant 
2.  Photocopy Identity Card of parent(s) / guardian(s)
3.  Certificates/Diplomas (if any) - PMR/SRP/SPM/STPM and etc
4.  Proof of income (parents/guardian) for subsidy or scholarship applications
